If you are not eligible to sign up for prescriber consultations with MC3 but want to participate in our educational opportunities, please complete this form and we will add you to our Educational Opportunities email list.

Once you fill out this form, if you indicate that you work in Wayne, Oakland, Macomb, Genesee, Ingham, or Washtenaw county, we will also send you information about our Perinatal Patient Care service and how to refer patients to it. 

Please note, if you are a prescriber and already signed up for MC3 consultation, you don’t need to sign up again here as you will automatically receive communications regarding MC3 education and do not need to complete this form.
