Consult Request

Our goal is to provide consultations efficiently and effectively. When you initiate an MC3 consultation, please have the following patient information ready:

  • Initials
  • Date of birth
  • Insurance (for purposes of identifying local behavioral health resources)
  • Additional questions include: current medications and past medication trials; psychiatric and social history; relevant medical/surgical information; and risk concerns

The more information that you are able to give, the better resources and consultative guidance we can provide. 

Please note that MC3 provides consultation to health care providers treating behavioral health in children, adolescents, young adults through age 26, and women who are contemplating pregnancy, pregnant or postpartum (up to one year). For consultation requests beyond our scope, some additional programs we recommend are MLine (24/7 physician-to-physician consultations) and Michigan OPEN (substance use disorder-related consultations).
