Consult Request

Our goal is to provide consultations efficiently and effectively. When you initiate an MC3 consultation, please have the following information ready:

• Patient’s initials
• Patient’s date of birth
• Patient’s insurance (for purposes of identifying local behavioral health resources)
• The best way for you to be contacted (e.g. office backline, cell phone)
• Additional questions include: current medications and past medication trials; psychiatric, developmental and social history; relevant medical/surgical information; family psychiatric history; and risk concerns

We understand that you may not have all of the background requested, however, the more information we have, the better resources and consultative guidance we can provide. Thank you in advance for your preparation.

Please note that MC3 provides consultation to health care providers treating behavioral health in children, adolescents, young adults through age 26, and women who are contemplating pregnancy, pregnant or postpartum (up to one year). For consultation requests beyond our scope, some additional programs we recommend are MLine (24/7 physician-to-physician consultations) and the Michigan Opioid Collaborative (substance use disorder-related consultations).
