This secure online form takes 5–10 minutes to complete, depending on the complexity of the patient information.
You may submit the form anytime, but your consult will be returned during our business hours, 9 a.m. to 5 p.m. ET, Monday through Friday.
Submissions made after 4:00 p.m. ET on weekdays, on weekends, or on holidays will be addressed the next business day.
Upon receipt of your online consultation request, your regional Behavioral Health Consultant may contact you if additional information is required.
