Submit a Consult Request Online

A few details to keep in mind prior to submitting your online request for a consult:

• This secure online form takes 5–10 minutes to complete, depending on the complexity of the patient information.
• You may submit the form anytime, but your consult will be returned during our business hours, 9 a.m. to 5 p.m. ET, Monday through Friday.
• Submissions made after 4:00 p.m. ET on weekends or holidays will be addressed the next business day.
• Upon receipt of your online consultation request, your regional Behavioral Health Consultant may contact you if additional information is required.
