Sign up for MC3
If you are a primary care provider, OB/GYN or psychiatrist who treats youth and/or perinatal women in the state of Michigan and you would like to utilize MC3 consultation we ask that you first sign up for our program.
Once you have filled out the form, a program coordinator or behavioral health specialist from our team in your region will reach out to you to welcome you to the program.
Please check the box if the statement is true for you. Both are requirements.
You are unfortunately not eligible to enroll in the MC3 program.
