Consult Request

Welcome to MC3 school-based consultation. Consult requests can be submitted using our secure online form below.

After you submit your request, someone from our team will reach out to you directly within 1–2 business days to schedule your consultation. MC3 school-based consultations are typically scheduled between 12–2 p.m. ET on Tuesdays and Wednesdays. 

In order to submit a consult request, you must be a school-based professional, including school therapist, social worker, psychologist, or school nurse, working in an ISD that has an agreement with MC3. If you work in an ISD in Michigan that is not yet participating in MC3 consultation and are interested in learning more, contact us at [email protected].

If you are requesting an MC3 Huddle, which is a consultation that includes the student’s primary care physician or other treating providers, please be sure to have the student’s parent or caregiver sign a release including the school, treating provider, and MC3. A release that includes both HIPAA and FERPA is preferred, such as our Release of Information (view an example completed Release of Information for guidance).
